Registration & Withdrawal

Registration is all Online:

Prior to enrolling your student as a Mustang, parents must register online by clicking HERE.

  • Please make sure that you have completed the Records Release Request form with all the information for the previous school attended.

Registration Process:

Once the online registration is complete, the parent/guardian should make a registration appointment by contacting our front desk – 770.381.9680

If you do not have your student’s transcripts, complete the Records Request form from MHS front desk and fax it to the previous school prior to requesting a registration appointment. If the Records Release Request form has not been completed correctly or is missing information we will not be able to obtain the necessary records and will not be able to schedule an appointment.

Once appointment time is solidified, parents/guardians are asked to come to the front desk to register their student with the following documents in hand:

  • Birth Certificate
  • Parental/Guardian State issued ID
  • Proof of Residency (Mortgage Deed or Lease Agreement and One utility bill)
  • If the student’s family is residing in the home or apartment of another individual, the following is necessary for enrollment:
    • Notarized third-person affidavit of residency
    • Two forms of proof of residency for person with whom family is living
  • Immunization Records (Georgia Form 3231 and Vision, Hearing, & Dental and Nutrition Screening – Georgia Form 3300)
  • Previous School Records
    • Report Cards or Official School Transcript

    • Withdrawal form from previous school
    • Discipline records for students enrolling in grades 7-12

Once all documents are verified, the registration clerk will receive the documents for student enrollment.

Parents and students will meet with an MHS Counselor to receive their academic advisement.

Students will be asked to report to the school’s Welcome Center the next instructional day to take our school tour.

Once the school tour is complete, the student will receive their schedule and will be escorted to class.
